Cardiac contractility modulation induced left ventricular reverse remodelling in a patients with dilated cardiomyopathy

Summary
Cardiac contractility modulator (CCM) pacemaker was implanted for a 65 years old man because of heart failure with reduced ejection fraction (EF: 30%), dilated cardiomyopathy, short QRS duration and NYHA III symptoms despite of optimal medical therapy. Due to CCM, reverse cardiac remodelling was induced, indicated by decreased left ventricular (LV) end-systolic volume (>15% of decrease), increased LV function (postoperative EF: 46%) and enhanced functional capacity (NYHA I). CCM implantation might be indicated for heart failure patients with LV EF of 25-45%, NYHA II-III clinical stage despite of optimal medical treatment, ischaemic or non-ischaemic etiology, who were not eligible for CRT implantation or previously proved to be CRT non-responder.
