The importance of adherence and persistence in oral anticoagulant treatment

Summary

In patients with atrial fibrillation, appropriate anticoagulant treatment is inevitable for effective stroke prevention. Due to the proven and significant advantages of direct-acting anticoagulants, the use of vitamin K antagonists has gradually been replaced. However, in clinical practice, in addition to the results shown in randomized clinical trials, the treatment efficacy is greatly determined by the persistence and adherence to the therapy. In our comprehensive review, we aimed to present the different anticoagulant treatments. Furthermore, we identified the factors influencing adherence and searched for strategies to improve it. Based on our findings, to achieve long-term effective stroke prevention in addition to choosing the optimal anticoagulant therapy, it is recommended to take into consideration the tolerability of the treatment and to select a personalized therapy that respects the needs of the patient.
